3% discount to military families!

Siblings of students also receive a 3% discount. Each qualifying family is limited to no more than a 6% discount.


A birth certificate must be shown for students new to our school.

  3. Verification of Birth Certificate

  4. Student Information Form

  5. School Entrance Health Form – current, physician-signed
    (MCH 213 G reviewed 3/20/2014)

  6. Immunization Record
    (May be printed from official online record such as My Chart™)

  7. Allergy Documentation – physician-signed

  8. Alternate Transportation Form

Please complete one set of forms for each child you are registering and attach a check made out to “All Saints’ Day School” for the Registration/Materials Fee (call to inquire current fee).
